This study is being conducted at The Sophie Davis School of Biomedical Education through the Rudin Research Fellowship. This study examines health risk behaviors among American Muslims between the ages of 12 and 24 currently living in the United States. The information collected will be used to determine the health needs of young American Muslims.

Participation in this survey is completely voluntary. The survey contains questions on demographics, religious practices, depression, suicide, smoking, alcohol, drugs, and sexuality.

The researcher will do everything possible to ensure that the responses are kept confidential; this website is secure, and uses the same technology used by banks and online merchants to safeguard your data. To further ensure your anonymous participation in this survey, it is suggested that you complete the survey on a public computer; use of a public computer will further protect your privacy since the computer identity can not be linked specifically to you or any other individual who uses that public computer. For added privacy, you may choose to erase the webpage usage history (known as “clearing the cache”) from your computers after the survey is complete to prevent others from knowing that you have visited this website.
